The Seagrape Plan

The Seagrape Plan for adults

The plan covers routine visits for one set amount a year. It is the practice's own membership, not insurance, and it can't be used with insurance. The price below is an example the practice replaces with its own.

    Yearly fee: ask the office

    Routine care for one adult, paid once a year, with no insurance involved.

    • Two exams and two cleanings a year
    • Routine X-rays, when Dr. Barksdale says you need them
    • A written estimate before any other treatment
    • Treatment beyond the routine visits
    • Insurance coverage of any kind
    Term:
    12 months, renewing automatically each year unless you cancel.
    Cancellation:
    Cancel by phone, email or at a visit before the renewal date. We send a reminder 30 days before.
    Refunds:
    Cancel within 30 days of joining and we refund the price, less any visits already used.

What the plan terms mean

Four lines on the card decide whether the plan suits you. Here they are in plain words.

  • Term

    The plan runs for 12 months from the day you join, and the price is paid once.

  • Renewal

    It renews each year by itself unless you cancel before the renewal date. We send a reminder 30 days before.

  • Cancelling

    Call, email or tell us at a visit. You can leave any time before the plan renews.

  • Refund

    Within 30 days of joining, we refund the price, less visits already used. After that, you keep the visits until the term ends.

Can my family join?

The plan on this page is for one adult. Ask us about anyone else in your household; we'll quote it in writing.

Does it pay for a crown or a filling?

No. The plan covers the routine visits it lists. Other treatment is quoted in writing before we start.

Can I use it with insurance?

No. It isn't insurance and can't be combined with it.
